I am petrified of the dentist?

I am 11 years old will be 12 in a month. I am SO scared of the dentist. I've had a horrible experience of dental issues. I am pretty sure i have 3 cavities. One is extremely bad, it is chipped off! Food gets stuck in where it is chipped so i get it out with a toothpick and then when i do it for like 30 seconds it starts to bleed. Do you think i will need a rote c or something? I have no idea why but i really just want to be put to sleep and they can do everything on my mouth and it will all be over. I also have braces, i've had them for 2 years now. My mom doesn't even know that i have 3 cavities. I am completely petrified. I am so scared of the needles - its my worst thing i hate about the dentist. It hurts so much! There are all like oh its only gonna be a pinch and it feels like it is going through my cheek!!! Please help me!!! When i think about the dentist for at least 5 minuets, i start to cry! Please just help me i have no clue what to say or do or anything! Thank You!
